Why do you ask about my ethnicity and race?
This is a CMS (Centers for Medicare and Medicaid Services) requirement that all health organizations are required to document to maintain their CMS certification. Johnston Endoscopy Center is both a CMS and AAAHC (Accreditation Association of Ambulatory Health Care) certified center.
Why do you ask about advanced directives?
This is another CMS and AAAHC requirement.
How long is my procedure going to take?
Why am I required to fill out these forms? Why don’t you have access to my information that the Dr. already has?
Johnston Endoscopy Center is not affiliated with the 5 practices that utilize our ASC and are not on the same EMR (electronic medical record) system. The Health Insurance Portability and Accountability Act of 1996 (HIPAA) rules and IT security guidelines do not allow us access to your information on the practices EMR. This is also why we cannot accept the forms via email. Email is not an approved format for submitting PHI (Protected Health Information) unless the email has the ability to encrypt the data shared and most personal emails do not have this functionality.

This will depend on a few things:
- The type of procedure you have – Go to Our Procedures
- Your insurance coverage
- Your Anesthesia Plan of Care received during the procedure
